FW& Parks, Event Committee, & Officials of every claim, demand, action, or right of action, of whatever kind or nature, either in law or in equity, arising from or by any bodily injury or personal injuries, known or unknown, death or property damage resulting or to result from any accident which may occur as a result of participation in the event, whether by negligence or otherwise by ourselves, our heirs, executors, or assignees. This release contains the entire agreement between the parties hereto, and the terms of this release are contractual and not a mere recital. I also understand that photographs may be taken of me and used for promotional purposes without royalties. Your submission of this entry form signifies you have read and agree to the above statement.

You should receive an ID number via email from Cindy if your entry is sent a week or so before the event. After that you can still enter with this form, but you may not receive a return email with an ID number because we will be the field. However your entry will still be valid.

Note: Anglers run the risk of not receiving awards if the mailing address and telephone information is not complete in the entry form.
Entries continue to be taken up to the last day of the event.
ID number's will be sent to you if you provide your e-mail address. Others will receive an ID number when they enter lake trout at the check-in stations if we have your entry before the weekend begins. Entries are taken at any time. You may even enter when you bring in your fish at the end of the day. There is an Early Bird drawing for entries received on or before September 21st.
